Reordering Death Certificates

Most funeral homes will be able to reorder certified death transcripts from the town registrar or county clerk's office in the location that the death occurred. NY State copies impose a $10.00 fee per copy.  In a case where the death occurred within the five boroughs of NY City, the certified copies impose a $15.00 fee per copy and may be ordered on-line via the internet at: www.nyc.gov  by mail or in person at:
